Stumbled upon this within my first playthrough in the newest update. My companion was slain and I am sure of it because if he was knocked out he would've gotten up (witnessed this earlier, he drops his weapon when he's downed) and I couldn't have looted him like any other corpse. So I continued alone, managed to get to the second level and thought "I wonder if I could go back to the first level" so I tested it before continuing and once I transitioned back to 2nd level... he pops out of nowhere. Watch the video and you'll see what I'm talking about. The save starts from right before I transition the first time (you can see the UI flashing in the video before 2nd floor loads).
